The National Institute for Health and Clinical Excellence have published guidance on Identifying and supporting people most at risk of dying prematurely. Included is a quick reference guide. It aims to identify what works in driving down population mortality rates in disadvantaged areas, with particular reference to proactive case finding and retention and access to services.
